Chemo & Infusion

Nursing documentation and billing that start from the dose given.

Infusion Nursing

The Medication Administration Record

Infusion nurses record what was given in the medication administration record (MAR), in the same chart where they can see what the provider ordered and which lab results came back.

What the nurse documents is what billing starts from, so nobody has to re-key it later.

  • Orders and Results in ViewNurses see the provider's orders and the lab results in the patient's chart.
  • A Record of What Was GivenEach administration is documented in the MAR.
  • Handoff to BillingThe documented dose and vial carry straight into charge preparation.

Billing Based on Dose and Waste

Billing Units Worked Out From the Dose and the Vial

Chemotherapy billing depends on details that are easy to get wrong by hand: how many billing units the dose comes to, and whether part of a single-dose vial was discarded.

Northstar's billing engine works these out from what the nurse documented. Anything that doesn't add up is flagged as an exception for your staff to review. Your team stays in charge of every decision.

  • HCPCS Units From the Dose GivenBilling units are calculated from the documented dose, not typed in by hand.
  • JW and JZ Waste ModifiersDiscarded drug from a single-dose vial is reported with JW; when nothing is discarded, JZ is applied.
  • NDC to HCPCS CrosswalkThe drug product used (its NDC) is matched to its billing code.
  • Exceptions to Your StaffAnything unusual is flagged for review instead of going out unchecked.

What Are the JW and JZ Modifiers?

JW and JZ are Medicare billing modifiers for drugs supplied in single-dose containers. JW reports the amount of drug that was discarded. JZ attests that none was discarded. Medicare has required JW for discarded amounts since January 1, 2017, and has required JZ when nothing is discarded since July 1, 2023.1

Since October 1, 2023, Medicare has returned claims for these drugs as unprocessable when the required modifier is missing.1 Northstar applies JW or JZ from the dose and vial the nurse documented, so the modifier matches the chart.

Why Does Drug Waste Reporting Matter?

Discarded drug from single-dose vials is a national cost issue in cancer care. The National Academies of Sciences, Engineering, and Medicine examined it in a 2021 consensus report, Medications in Single-Dose Vials: Implications of Discarded Drugs.2 Members of the study committee summarized it for clinicians in JAMA.3

For an infusion practice, the practical part is getting each claim right: billing units that match the dose given, and a waste modifier that matches the vial used. That's the work Northstar's billing engine prepares for your staff to review.
